# Architecture decision records — claude-ops-skills
OKF bundle (one decision per file, `type: ADR`). Numbers are creation order
and permanent. A superseded or amended decision keeps its file with a status
note and a pointer; never delete. `verified:` appears only where a claim was
checked against its source rather than recalled.
